It's worth noting that friendly urls don't do much good unless one uses keyword-rich page titles.

I am completely agree with the above post that these are very common mistakes which most of the SEO are avoid because these can become a very big when you are trying to get the ranking for your website. There are many experienced SEO guys out there which are taking care of these issues but after some times they just ignore then and latter these problem spoil their rankings and some may cause the traffic drop. So i think we all should take care of all these issues and should try to avoid then when every we optimizing a website.
