Who says there are so few leaders? The only people who can call a person a leader are the followers. People outside the organization might call a person a leader, yet she might have just been a dictating tyrant who used her power of authority to get things done

If you are referring to Mistakes, I am not sure if we can nail it down to one thing. Compromising Integrity would be high in my list. The first one is pride, that the leader thinks that they know the answer and therefore they then are endowed with the responsibility, the right to act without regard for others. Certainly that is something I struggle with.
